Before visiting the clinic, in order to shorten your waiting time, we ask that you please download and complete our patient registration forms: available in English or Spanish. 

On the day of your appointment, please bring, if possible:

1. The completed registration forms
2. A list of medications and supplements that you are currently taking and their dosages
3. Copies of your medical history, x-rays, and/or lab reports if you have them
